January 15-16 2025. Please sign up online
here by December 20th if you don’t have a family dentist and would like your child to participate in this program.
Parents of
Kindergarten, 2nd and 6th grade students: Please note that by
May 15th you must show proof that your child was seen by the dentist within the past 18 months. This is a requirement for all children in the State of Illinois. If the
