Reminder for Parents of Incoming Sixth Graders
A packet was mailed home in March regarding health requirements for entry into sixth grade. A current physical examination (dated 8-15-23 or later) is required as well as proof of receiving 1 dose of Tdap, 3 doses of hepatitis B vaccines, 2 doses of varicella vaccines and 1 dose of meningococcal conjugate vaccine. The State of Illinois Certificate of Child Health Examination form should be completed by your health care provider including documentation showing proof of required immunizations. Complete the parent portion on the physical form by answering Yes or No to every question, then sign and date where indicated. It is highly recommended to submit these requirements to the school nurse/office on or before the first day of school or via email to [email protected]Sixth grade students who do not have the above requirements completed will be excluded in the fall. In addition, proof of dental examination is also required which you may submit anytime during the school year.

A current physical examination (dated 8-15-23 or later) is required for entry into Kindergarten. All children entering Kindergarten must also show proof of complete immunizations with required intervals including Dtap (4 doses), Polio (3-4 doses), Hepatitis B (3 doses), MMR (2 doses) and Varicella (2 doses). The State of Illinois Certificate of Child Health Examination form should be completed by your health care provider including documentation showing proof of required immunizations. Complete the parent portion on the physical form by answering Yes or No to every question, then sign and date where indicated. It is highly recommended to submit these requirements to the school nurse/office on or before the first day of school or via email to [email protected]Kindergarten students who do not have the above requirements completed will be excluded from school in the fall. In addition, a current vision examination and dental examination are also required.
